Friday, 8:30 a.m. to 4:30 p.m. If you have an emergency after hours or
on weekends, your call will be answered by our answering service. They
will notify the provider on call of your urgent concern.

PRIVACY
& HIPAA COMPLIANCE
The
Health Insurance Portability and Accountability Act (HIPAA) requires
that we notify patients of our privacy practices. Below are two downloadable

APPOINTMENTS
Please plan to arrive at our office at least 20 minutes before your
scheduled appointment time. This allows you to provide us with your
up-to-date insurance information or any changes in your medical history.
We will also provide you with information about the Health Insurance
Portability and Accountability Act (HIPAA) and request your review and
signature to acknowledge your receipt of the information.
Cancellation
Policy - If you are unable to keep your appointment, please
contact our office as soon as possible. We require a minimum of 24 hours
advanced notice to avoid a $25 cancellation fee.
If you are coming to our office for a consultation and you or your primary
care physician suspect you may need a surgery procedure, we will ask
you to return to see us for the procedure. This allows us to schedule
the amount of time needed to do the procedure and it allows us to contact
your insurance if authorization is needed.
Complimentary
appointments are available with our Skin Care Specialists to discuss
skin care products,
chemical peels
or laser services. It
is not necessary to have been a prior patient in our practice nor is
it necessary to have seen another provider in order to receive laser
treatments.
Our office carries skin care product lines for all types of skin as
well as sunscreen and lip balm. Please contact our staff at 319-1100
for more information.
REFERRALS
While many HMO and EPO Insurance Plans have improved their referral
process, several still require a document such as a prescription from
your primary care physician. Please have your referral sent to us prior
to your appointment or bring a copy with you to your appointment. If
we do not receive your referral by the time of your visit, you will
have the option to sign a release allowing us to bill you, or to reschedule
the appointment after the referral is available. We appreciate your
help in making sure we receive a referral so that you are not responsible
for the bill.
REGISTRATION AND HISTORY INFORMATION
We ask that you arrive 20 minutes early for your initial appointment.
You will be asked to fill out a registration form and a history form.
On return visits we will ask you to review and update the information
for any changes.
PRESCRIPTION
REFILLS
For prescription refills, please have your pharmacy call or fax us the
request. If you have changed pharmacies, please let us know where you
would like your prescriptions filled. This allows us to update your
medical record with the current information.
INSURANCE
We see patients from many insurance plans. Please bring your insurance
card with you, and we will scan it into our computer system. We know
it is not always easy for you to know everything about your insurance.
We will work with you to ensure we are following the right process for
your insurance company. We also ask that you notify us if your insurance
changes at any point during your visits with us.
PAYMENT
We collect co-payments at the time of your visit as required by your
insurance plan. Please be prepared to pay your co-payment when you come
to see us. If you are paying for your own health care, we request that
you pay for your office visit on the day of service. For your convenience
we accept cash, check or credit card (Visa, MC, American Express and
Discover.)
